Meet Our Providers
Dr. Joel Cohen MD, co-founder of Elevate Hospice has provided compassionate and quality medical care in Arizona since 2005, caring for frail and complex geriatric patients, providing palliative and hospice care, as well as mobile advanced wound care treatment. His practice approach emphasizes educational focus, evidence-based as well as patient-centered medical care. His published educational writings and two books convey his commitment to education and prevention.
Anneli Kumpula, NP, co-founder of Elevate Hospice, is a board-certified nurse practitioner with extensive experience in both hospice and psychiatric care. Her clinical background includes providing compassionate end-of-life support and addressing complex mental health needs, ensuring holistic care for patients and their families.
