Considering hospice sooner in the disease process will help the patient and their loved ones with their unique physical, emotional, spiritual, and financial needs. The skilled professionals at Elevate Hospice and Palliative Care provide much-needed help and support, developing a plan of care together with the patient’s own physician and family.
Most health insurances, including Medicare and Medicaid, provide full coverage for hospice care. Usually, with such coverage there are no co-payments or deductibles, or few, if any, out-of-pocket expenses. Patients who are underinsured or who have limited resources may qualify for financial assistance, which is funded by community donations.
Elevate’s Hospice care is available 24 hours a day, 7 days a week. We recognize that the decision to elect hospice care is a difficult one and there are a lot of emotions and uncertainties for you and your family. We consider it a privilege to be chosen to participate in this journey with you.

Hospice is designed to provide supportive care and services to persons who have a life-limiting illness. The focus of hospice is on the patient’s comfort and quality of life, and to provide relief for the entire family and caregivers from the burdens typically associated with this time of life.
