Grief & Bereavement Support

Losing a loved one is never easy. Our bereavement team is here to support you through the grieving process, offering counseling, check-ins, and grief resources for up to 12 months (or more) after your loved one’s passing.

Respite Care

Caregiver burnout is real. Respite care offers short-term relief, allowing you to rest and recharge while we care for your loved one in a certified facility for up to 5 consecutive days. You deserve a break - we’re here to make that possible.

When Is It Time for Hospice?

Knowing when to transition from curative treatment to comfort-focused care can be overwhelming. If your loved one is experiencing frequent hospitalizations, worsening symptoms, or increasing dependence on daily care, it may be time to consider hospice.

The Elevate Hospice care team is made up of compassionate professionals who work together to provide physical, emotional, and spiritual support to family caregivers:

Chaplains
  • Offer spiritual and emotional support for caregivers and families

  • Provide counseling, prayer, and comfort during difficult times

Social Workers
  • Help families navigate decisions, insurance, and practical matters

  • Provide emotional support, counseling, and caregiver resources

Counselors
  • Grief counseling during and after a loved one’s passing

  • Lead support groups and offer one-on-one guidance for emotional healing

Volunteers
  • Provide companionship to patients, giving caregivers time to rest

  • Assist with light household tasks or errands to reduce caregiver stress
