When it comes time to face the many difficult end-of-life decisions, choosing a qualified caregiving team for your loved one need not be one of them.
Whether it’s through hospice care or palliative care – we understand the need for our services marks a new chapter in life. With so many emotions and decisions ahead, you need a team who understands what you're going through. We empower our patients and families during this most vulnerable time of life by providing them with information and support to allow them to make decisions that enhance their quality of life.
We are passionate about raising the standard of hospice care, starting with our nurses whose schedules allow them time to provide elevated care, in-home visits from our Medical Director, aides who are passionate about creating meaningful experiences with our patients, and social work and spiritual care that is personalized for each patient's unique needs.
Patient focused comfort-care.
Enhanced quality of life for patients.
Grief support for the entire family.
As witness to so many of our patients' end-of-life experiences, we have learned much about the importance of human connection. Years of providing support, holding hands, trading laughs, and sharing comfortable silences have brought empathy to the forefront of our care process. Our individualized care plans go beyond physical needs. They are designed to improve quality of life for patients and families.
Discuss with your Doctor or with the Elevate team to determine if you are eligible for a Hospice Referral and to make informed decisions about your care.
You can take the initiative and request an evaluation at any time. We will contact you to discuss your eligibility and next steps. Hospice care begins with a referral, usually from your physician, but referrals can be made by you, family members or even friends.
"...you were an angel to us in our time of need. We will forever remember your care and support during our dad's final days."
"We can't thank you enough for the help and support you provided us. Your entire team was so caring and responsive. We were able to spend quality time with mom instead of worrying about her medical needs."
I have to tell you we are very impressed with you and your hospice team. Thank you very much.
