Raising the Standard in Hospice Care

When it comes time to face the many difficult end-of-life decisions, choosing a qualified caregiving team for your loved one need not be one of them.

Whether it’s through hospice care or palliative care – we understand the need for our services marks a new chapter in life. With so many emotions and decisions ahead, you need a team who understands what you're going through.  We empower our patients and families during this most vulnerable time of life by providing them with information and support to allow them to make decisions that enhance their quality of life.

We are passionate about raising the standard of hospice care, starting with our nurses whose schedules allow them time to provide elevated care, in-home visits from our Medical Director, aides who are passionate about creating meaningful experiences with our patients, and social work and spiritual care that is personalized for each patient's unique needs.

Keeping Hope in Hospice

Caring nurse with patient
As the end of life approaches, hope changes form -- from cure to treasuring each day. Mending broken relationships or reuniting with forgotten friends may be a priority.

Reassurance that there will be relief from pain and suffering offers hope and enables people to enjoy better quality of life.

Short-term goals in palliative care can be achieved including finding meaningful activities, and checking off valued and achievable accomplishments on a wish and goals list.

Other valued activities for those approaching the final stage of life include creating a formal legacy, engaging in life review, and finding meaning each day, even as people face incurable and progressive chronic illnesses.

3 Simple Steps to Elevated Care

As witness to so many of our patients' end-of-life experiences, we have learned much about the importance of human connection. Years of providing support, holding hands, trading laughs, and sharing comfortable silences have brought empathy to the forefront of our care process. Our individualized care plans go beyond physical needs. They are designed to improve quality of life for patients and families.
