We are passionate about raising the standard of hospice care, starting with our nurses whose schedules allow them time to provide elevated care, in-home visits from our Medical Director, aides who are passionate about creating meaningful experiences with our patients, and social work and spiritual care that is personalized for each patient's unique needs.

Discuss with your Doctor or with the Elevate team to determine if you are eligible for a Hospice Referral and to make informed decisions about your care.

You can take the initiative and request an evaluation at any time.  We will contact you to discuss your eligibility and next steps.  Hospice care begins with a referral, usually from your physician, but referrals can be made by you, family members or even friends.
