Elevate Hospice is led by the clinicians who founded it. Nurse Practitioner Anneli Kumpula and Physician Joel Cohen are directly involved in guiding patient care every day. Based in Phoenix, they work alongside the care team to ensure every patient and family receives thoughtful, personalized support. This hands-on leadership means decisions are made quickly, communication is clear, and care stays focused on what matters most.

We are passionate about raising the standard of hospice care, starting with our nurses whose schedules allow them time to provide elevated care, in-home visits from our Medical Director, aides who are passionate about creating meaningful experiences with our patients, and social work and spiritual care that is personalized for each patient’s unique needs.
Our passion is to provide our patients and their families with quality of life and comfort through this emotion-filled time.

Keeping Hope in Hospice

Discuss with your Doctor or with the Elevate team to determine if you are eligible for a Hospice Referral and to make informed decisions about your care.
Request An Evaluation Or Referral
You can take the initiative and request an evaluation at any time. We will contact you to discuss your eligibility and next steps. Hospice care begins with a referral, usually from your physician, but referrals can be made by you, family members or even friends.
